What Are You Volunteering For?
You may be required to complete multiple forms depending upon the role you volunteer for. See the guide below.
No volunteer should begin any games/practices or other volunteer activity until they have completed the proper applications and registrations.
A
Adult Volunteer Application
All volunteers, including but not limited to, Board Members, Referees, Coaches, and Assistant Coaches participating on a Tewksbury team affiliated with Middlesex Youth Soccer League, Massachusetts Youth Soccer Association or Tewksbury In Town Recreational Leagues must register as an Adult Volunteer.
This application will take you to the Massachusetts Youth Soccer Association website. Create or login to your existing account and complete the steps to the background check (CORI). Once the background check is complete, you will receive a confirmation email. Please follow the instructions in the email to finalize your Application.
Please note you must submit a Massachusetts Youth Soccer Association CORI form each year even if your account has a verified CORI status. This allows TYSL to print your coaching credentials.

Coach Registration (FALL '24)
This registration form will add coaches and assistant coaches to our TeamSnap Database. You will be asked for basic contact information and prompted to pick the Program you are volunteering to coach in. If you are coaching or assistant coaching on more than one team you will need to submit this form again under each Program.
Please be informed that there will be prompts about fees but there will be zero balance at checkout and you will not be charged.
Once the ADULT VOLUNTEER APPLICATION is completed and a CORI status is verified, League Directors will then assign coaches to teams.

Travel - NEW Head Coach Application
If this is your first time coaching in TYSL's Travel League you must complete this form. Returning Travel Coaches do not need to complete this. However, both new and returning must fill out the Coach Registration Form for the current upcoming season.
Besides our kids, our Volunteer Coaches are our most valuable asset. Thank you for volunteering your time for the benefit of our kids.
In completing this application, please be as candid as possible. There are no right or wrong answers. Please note that all volunteer positions with the TYSL are conditional upon completing a CORI criminal background check which is part of ADULT VOLUNTEER APPLICATION. Additionally, All coaches will be required to maintain a
- Heads Up Concussion Training Certification
- Abuse Prevention Certification
All coaches are encouraged (but not required) to attend a Grass Roots training class in the format most applicable to their age group (7v7, 9v9, or 11v11). Successful completion of these certificates is reimbursable to TYSL for all coaches and assistants based on TYSL policies.

Under 18 Volunteer Registration
This registration form will add Under 18 Volunteers to our TeamSnap Database. You will be asked for basic contact information and prompted to pick Under 18 Volunteer.
